Formal application to compete in the Russell Stover Classic on January 4-6, 2008. It is certified by the undersigned that all participants agree to all of the rules and regulations governing this tournament. It is also certified by the undersigned that all players on the registered roster were born on the dates recorded and are eligible to compete in competition in the 2006-2007 season and have been signed to an USA Hockey registration form. It is understood that no exception to the rules will be tolerated.
